Our Intake Process

If you are a potential client needing pre and postnatal midwifery care, you can either:

1. Self-refer by completing our online Intake Form;

   or

2. Have a care provider / support person (i.e. social worker, NP, etc.) complete our online Intake Form on your behalf;

   or

3. Have your primary care provider (i.e. GP, NP) refer you to our clinic by writing a referral letter. *Referral letters must be submitted to our office via fax or email.

If you are a potential client needing only postpartum care (i.e. midwifery postpartum and newborn care from birth to 6-8 weeks), you can:

• Self-refer by completing our online Postpartum Intake Form;

  or

• Have your health care provider complete our Postpartum Referral Form *completed forms must be submitted to our office via fax or email.

If you are interested in receiving:

• prenatal and/or postpartum care from Huckleberry Midwives;

• postpartum care from netəł birthwork

Please follow the same intake process as outlined above.

Lactation support services for babies less than 6 weeks old are covered by MSP with a referral from a health care provider.

If you are needing infant feed / lactation support from our team:

• have your health care provider (midwife, physician, registered nurse, nurse practitioner or social worker) complete our referral form.

Please note, there is no self-refer process for infant feeding support at this time.
